Adding Forms to Form Sets

Add evaluation forms to your form sets and configure their weights and scoring.

Form sets serve as evaluation instruments that contain one or more forms. You can add existing forms to your form sets and configure how they contribute to overall scoring.

  • system_administrator or organization_administrator role
  • At least one existing form set
  • At least one existing form to add
  1. Navigate to System Admin → weCLIMB → Form Sets
  2. Click on the form set where you want to add a form
  3. In the form set detail view, click Add Form
  4. Select the form you want to add from the dropdown list
  5. Configure the form settings:
    • Display Order: Set the position of this form within the form set
    • Include Score: Check this to include the form’s score in the overall form set score
    • Weighted Value: Enter a decimal weight between 0 and 1 (e.g. 0.5) if using weighted scoring
  6. Click Save

The form will now appear in the Forms section of your form set detail page.

Forms within a form set are presented to evaluators in the order you specify:

  1. In the form set detail view, locate the Forms section
  2. Each form shows its current display order number
  3. To reorder forms:
    • Click Edit next to a form
    • Change the Display Order value
    • Click Save

Lower numbers appear first in the evaluation interface.

When your form set contains multiple forms, you can weight their contribution to the overall score. Weights are set on each form’s Include Score / Weighted Value connection to the form set (not on the form itself).

Rules the system enforces:

  • Weighted values must be greater than 0 and less than or equal to 1
  • If you add a weighted value to any scorable form, all scorable forms in the form set must have a weighted value — you cannot mix weighted and unweighted forms
  • The weighted values of all scorable forms must add up to exactly 1
  • Forms where Include Score is off, or that contain no scorable questions, cannot carry a weighted value — the system will clear any value set on them automatically

If scoring weights are not configured, the system calculates a simple average across the scorable forms.

Form sets can contain multiple forms to create comprehensive evaluation instruments:

  • Sequential Evaluation: Forms are presented in display order during observations or performance reviews
  • Focused Assessment: Use different forms to evaluate different aspects of performance
  • Flexible Scoring: Weight forms differently based on their importance to the overall score

To remove a form from a form set:

  1. Navigate to the form set detail page
  2. Locate the form in the Forms section
  3. Click Remove next to the form
  4. Confirm the removal

This removes the form from the form set but does not delete the form itself. The form remains available for use in other form sets.

Form set visibility is determined by which organizations have access to the forms contained within the form set — not by a separate organizations tab on the form set itself. A form set is visible to an organization when at least one of its forms has been assigned to that organization (or an ancestor organization) through product assignments.

To control which organizations can access a form set, manage form assignments at System Admin → Organizations → [Organization] → Forms.

  • Clear Naming: Use descriptive form names to help evaluators understand the purpose
  • Logical Ordering: Arrange forms in a sequence that makes sense for the evaluation process
  • Consistent Weighting: If you weight any scorable form, you must weight all of them — plan your weights before saving so they sum to 1
  • Non-Scorable Forms: Forms with no scorable questions can still be included in a form set; they simply won’t contribute to the score
  • Regular Review: Periodically review form combinations to ensure they meet evaluation needs

Your form set is now ready for use in observations and performance reviews within the organizations that have access to its forms.
